• Registration and evaluation forms for accredited versions

All you need to provide:

• Location
• Registration/sign-up staff
• Printed copies of the handouts for all attendees
• Advertising and refreshments at the facility's descretion
• A sound system and microphones if needed
• Transportation stipend, if out of the Phoenix area

Fees

Professional Package

• $20 per student, with a minimum or 10 students.
• Flat fee negotiable.

Memory Care Package:

• $200 flat fee.
• Free with purchase of LBD Trainer's Kit

Student Nurses Package:

• $150 flat fee.
• Textbooks available at minimum price.
